What Outstanding Musician Grant Award am I applying for?

Tuition for a private teacher for one year.
Other tuition assistance plus the loan of an instrument or bow (valued up to $5000).
Professional instrument grant award. This includes professional photoshoot, use of professional instrument for 1 year (up to 3 consecutive years of reapplication), featured artist in an FLC sponsored concert event, a recording of the artists prefer repitoire. The artist will receive a copy for their personal use.
